Job Summary:

The SCPM provides clinical expertise, leadership and coordination of all elements of the KFH Sunnyside and KFH Westside Stroke Care Programs (multidisciplinary stroke rounds, stroke research, Neuro-interventional Radiology stroke program maintenance, patient and family education, staff and provider education and community outreach), necessary to meet all National/Evidence-based stroke care clinical practice guidelines and the disease specific care certification (DSCC) standards employed at each hospital (i.e. Joint Commission Advanced Primary Stroke certification at WMC or DNV Comprehensive Stroke certification at SMC). The SCPM collaborates with the interdisciplinary stroke committees, EMS and operational stakeholders across the region to plan, organize and evaluate stroke care and maintain compliance with designated DSCC stroke program standards specific to each hospital. Ensures that stroke education is reliably incorporated into annual unit training plans and that stroke training is sufficient to promulgate evidence into practice. The SCPM co-leads with the Programs Medical Director both hospitals Stroke Teams. The SCPM Is the subject matter expert for the standards content (i.e. DNV, TJC, the Brain Attack Coalition, and the various American Stroke Association (ASA) Clinical Practice Guidelines) and guides education, documents development, performance improvement, project management, and assessment related to the organizations treatment of stroke patients.

Essential Responsibilities:


  • Leadership: The SCPM serves as the subject matter expert for the designated DSCC organization Stroke Program measures, clinical practice guidelines, and the American Stroke Association practice guidelines to ensure expert, comprehensive stroke care from the ED through discharge planning. Collaborates with the Stroke Medical Director and interdisciplinary stroke committee in the development and maintenance of the stroke program, stroke committees and work groups. Serves in a leadership capacity for all program related committees. Collaborates with the Medical Director of the Stroke Program in guidance of the stroke committees, teams, and work groups. Develops policy and procedure consistent with research and latest regulatory requirements. The SCPM organizes and monitors the process for multidisciplinary patient rounds to ensure consistent high quality, comprehensive stroke care, regularly attending at both hospitals to evaluate effectiveness. Works with medical director for medical staff follow up issues and with nurse managers for staff follow up. Collaborates with Hospital Quality team for chart abstraction, data analysis and data submission to the DSCC designated organization (i.e. DNV and/or TJC). Maintains knowledge of evidence-based practice changes, new research, and new recommendations from the designated DSCC organizations and the American Stroke Association. Reports program progress to Quality Council, and the Medical Executive Committee as requested.

  • Standards Compliance and Performance Improvement: Accountable to and maintains all necessary standards required to assure compliance with the designated DSCCs performance measures for stroke patient care and collaborates with operation leaders when a standard is evaluated as not in compliance. As such the SCPM works in collaboration with the stroke committee members to meet performance measures, identify performance deficiencies, and implement interventions to remedy issued identified by quality activities and ongoing audits. Consistent with the designated DSCC standards for performance improvement, the SCPM identifies opportunities, develops PI plans, and coordinates and leads the required number of specific improvement projects; uses improvement science to organize PI projects (e.g. Lean, IHI, 6 sigma or CQM/TQM methodology). Interprets data and oversees reports development and reports results to the interdisciplinary stroke committee, Quality Council and Medical Executive Committee meetings. Prepares for and leads the designated DSCC site visits as well as any mid certification evaluations (such as the TJC intra-cycle call).

  • Community outreach: Collaborates with members of the EMS system, including paramedics and dispatch to provide regular stroke education and feedback on stroke cases. Leads the stroke committees to develop and implement community outreach events for public education of stroke awareness as required to maintain standards compliance with the designated DSCC organization.

  • Education: Oversees stroke education delivery to ensure that impacted clinical staff meets the education requirements, as mandated by the DSCC organization. Collaborates with staff development, simulation teams, nursing leaders and the Medical Directors to organize clinical education and training activities for staff and providers who care for stroke patients in order to ensures care is delivered consistent with Clinical Practice Guidelines and remains in compliance to standards outlined by the American Stroke Association, American Heart Association, and Brain Attack Coalition in the care of patients who have suffered a stroke. Provides education to Emergency Department and Intensive Care Unit in best practice and timely stroke care including Alteplase delivery, post-Alteplase monitoring, interventional services, and management of real and potential complications. Educates and trains ancillary department employees in the specific expectations of primary and comprehensive stroke centers (i.e. turn-around time for results of CT scan, laboratory results, EKG, and door to needle times, etc). Collaborates in the education of members of the EMS system. Develops and maintain discharge education and instruction materials and maintains Stroke Central intranet site.

  • Long term planning: Provide leadership to maintain a comprehensive stroke program. Provide leadership for the development of community collaboration to support stroke patients and their families and care givers.
