Duties

SCHEDULING
Assists with scheduling of assigned area for staffing to include monthly schedule, overtime needs and
contacting staff for specific shift coverage.

LTC READINESS:
Is considered an expert in LTC readiness, assisting with training staff in preparation for LTC survey.
Must be proficient in policy and procedures within the GEC department and those which effect the
Community Living Center units.

PRECEPTOR:
Is a competent preceptor for the unit.

INDIVIDUALIZED RESIDENT CARE:
Performs nursing procedures appropriate to grade level approved by Nursing such as:
- Screening and data collection to perform and complete the admission process.
- Bladder catheterizations/irrigations.
- Bowel/Bladder programs and training.
- Enteral tube feedings including instillation of medications via enteral feeding tubes.
- Oxygen therapy to resident using cannula, mask, or tracheostomy.
- Respiratory treatments, including C-PAP and BI-PAP therapy.
- Tracheostomy care including suctioning and cleaning, inner cannula change.
- Complete ostomy care.
- IV therapy.
- Indwelling and in/out catheter placement.
- Phlebotomy
- Application and removal of simple to complex dressings on all types of wounds.
- Wound irrigations.
- Application and/or removal of braces, splints, and slings
- Postmortem care.

ADMINISTRATION OF MEDICATIONS:
Administers and documents medication, utilizing BCMA, according to the Medication and IV Fluid
Administration, Bar Code Administration, and Medication Administration policies and procedures.

MANAGEMENT OF INFORMATION:
The LPN carries out the assignment under the direction and supervision of the Nurse Manager or
Registered Nurse. Delegates assignments are based on acuity and needs of that assigned area.
- Administrative duties to include 24-hr report, answering phones, preparation of assignments
based on assigned area needs, and documentation for incident reports in JPRS.
- Participates in residents IDT meetings and contributes to the interdisciplinary team in the
development, implementation, and evaluation of the resident treatment plan. Works closely with
the interdisciplinary team members in the integration of the total resident care plan. Reinforces
plan of care with resident/family/caregiver to promote understanding of the goals.
- Recognizes the need for instituting emergency measures. Initiates CPR or the Heimlich
maneuver when such emergency measures are indicated. Must be able to initiate facility
protocols for fire/emergency situations independently.
- Transcribes verified medical orders according to established policy in the absence of a
Registered Nurse or clerical assistance or to meet workload needs.
- Delivers verbal report and/or written end-of-tour report on his/her assigned residents/units for the
use of the oncoming tour.
- Documents in the Electronic Medical Record and on any paper record form utilized in
accordance with nursing policy and procedure.
- Assures resident privacy, confidentiality and sensitive information is protected in accordance with
all agency regulations, policy, and computer access agreement.

OTHER:
The LPN must possess the knowledge and skills necessary for concentrated effort and self-direction in
carrying out the most complex practical nursing care assignments. The LPN must have the skills and
knowledge to perform independent clinical judgement without a readily available supervisor. The LPN
reports directly to the Nurse Manager and is guided by the registered nurse but maintains
independence when in the Team Leader role. The LPN Team Leader handles any problems or issues
within the area of work assignments in accordance with instructions, policies, and training. Completed
work is evaluated by the Nurse Manager or registered nurse per policies and requirements.
The LPN Team Leader demonstrates the critical skills and knowledge to provide care in relation to the
age/population needs of the residents served within a specific assigned area to include dementia,
rehabilitation, complex wound care, palliative and hospice care, restorative care, respite care, and long
and short-term skilled care. The LPN will understand the associated changes to residents with the
various medical complexities and be knowledgeable to implement critical thinking skills to provide
emergent care when necessary.

Work Schedule: Full Time, Days. M-F 7:30am-4:00pm. Weekends and holidays as needed. Flexibility is a must.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:
a. Citizenship. Citizen of the United States.

b. Education. Graduate of a school of practical or vocational nursing approved by the
appropriate State agency and/or accredited by the National League for Nursing
[Accrediting Commission (NLNAC) at the time the program was completed by the applicant.
Verification can be obtained from the State approving agency [or] the National League for
Nursing Accrediting Commission, 61 Broadway, 33rd Floor, New York, NY 10006.

c. Licensure. Full, active, current and unrestricted licensure as a [ ] licensed practical or
vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United
States, or District of Columbia.

d. Physical Standards. See VA Directive and Handbook 5019.

e. English Language Proficiency. Licensed practical or vocational nurses appointed to
direct patient care positions must be proficient in [both] spoken and written English as

Grade Determinations: The GS-7 grade level is for select, complex LPN/LVN positions, as established
by each health care facility. To be advanced, employees must meet all of the

Preferred Experience: Program management, broad knowledge base to develop and modify treatment plans working in collaboration with RNs and Nurse Manager. The full performance level of this vacancy is Grade 07. The actual grade at which an applicant may be selected for this vacancy is in the range of Grade 07 to Grade 07.

Physical Requirements: Functions require extensive periods of standing and walking. Working with
patients require regular and reoccurring bending, lifting less than 35 pounds, stooping, stretching and
similar activities. The employee may be scheduled to work varying hours of duty and may, on occasion,
