Job Summary:

The Cancer Care Navigator will oversee and coordinate all aspects of the integrated health care model for the KP multi-regional cancer clinics. The Nurse Navigator serves as a single point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services. The Nurse Navigator will achieve positive patient outcomes in collaboration with the multi-disciplinary specialty teams to ensure effective integration of the care continuum from diagnosis through survivorship.

Essential Responsibilities:


  • Assists patients in understanding their diagnosis, treatment options and resources available, including educating patients, coordinating appointments, tracking completion of plan of care through survivorship, in person support and assessment of care needs and distress related to cancer diagnosis. Initiating the Oncology history and treatment summary and Survivorship care plan.

  • Serves as an essential link between patients and all other care providers in coordination of care. The Navigator facilitates changes in the plan of care and brokering necessary services. Service settings may include Medical Centers, Home and Administrative Building. The care is assessed and conducted by phone and in person.

  • Facilitate appointments for consultation, supportive services and routine surveillance within established service standards.

  • Coordinate and standardize patient education tools across the cancer care service lines.

  • Practices within ethical guidelines using established policies and procedures for guidance. Abide by HIPPA guidelines to maintain confidentiality of patient records, information and departmental activities involving patients.

  • Coordinate weekly multidisciplinary meetings to discuss cases. Activities to include member outreach, case preparation and presentation, multidisciplinary provider conference and clinic session coordination.

  • Ensure direct access to health care services for cancer problems for all KPMAS members.

  • Consults an collaborates with the multidisciplinary team to facilitate a positive care experience for KP members.

  • Provides direct support during clinic sessions to include member education, facilitation of surgical scheduling and/or additional testing as directed by the multidisciplinary team.

  • Updates clinical documentation and tracking documents to include multidisciplinary conference minutes, Navigation tracking, Radiant and associated dashboards and patient lists.

  • Provides consultation and education to other KP staff regarding the referral process for member with cancer navigation needs.

  • Builds effective working relationship with MAPMG providers, hospital staff, vendors and all clinical departments. Works cross functionally with other department in striving to meet organizational goals and objectives.

  • Performs other related duties as directed.
