Job Summary:

Evaluates programs and services to meet the needs of patients with cancer. Case manages selected populations of patients diagnosed with cancer using variety of skills, psychosocial and education interventions. Functions as liaison among all departments and services providing care for patient. Educates members, patients, staff and community about cancer.

Essential Responsibilities:

  • Develop programs to improve the overall journey of the cancer patient; providing timely and coordinated care with a multidisciplinary approach. Areas of accountability include but are not limited to; patient navigation, peer navigation, timely access to care, psychosocial support, access to community resources, patient education and pain management.
  • Participates in development and implementation of programs for region aimed at early identification and treatment of patients with cancer in consultation with affected departments and practitioners.
  • Provides direct patient care and case management services to selected cancer populations to assure that patients receive the necessary services. Monitors on-going care in accordance with established standards. Tracks and follows-up on patients with abnormal screening results (i.e. mammograms and FOBT) to assure the deliverance of timely care. Collaborates with departments to assist with coordination of interdisciplinary services.
  • Orders testing (i.e. diagnostic imaging and laboratory) and referrals (i.e. dietitian, clinical pharmacist) to assist with patient care.
  • Initiates and participates in patient care conferences with other health care team members to discuss, coordinate, evaluate and improve care and services for individual patients.
  • Educates patients, members, staff and employer groups and community about cancer. Promotes prevention and screening activities related to cancer. Represents Kaiser Permanente at conferences and in designated community groups. Plans, develops and conducts professional development activities focused on cancer interventions.
  • Serves in a leadership role on regional and inter-regional committees; Cancer Committee, Patient Navigation Team, Safety Net Committee, Interregional Cancer leadership Group and others as needed.
  • Leads the development of standards, guidelines, policies and procedures. Coordinate and standardize patient education tools across the departments providing cancer care.
  • Identify and share interregional best practices to improve quality, service, reputation, and reduce waste. Integrate supportive care services as part of routine cancer care.
  • May perform patient care to the extent necessary to maintain clinical expertise, competency, and licensing necessary to fulfill job responsibilities and to direct the provision of care on the unit.
  • Provides direct patient care on an as needed basis. Provides services that are within scope of license and in compliance with all legal, regulatory, and policy requirements relevant to clinical role performed.
