Job Summary:
Ensures smooth transition of patients from other levels of care to Skilled Nursing Facility (SNF). Coordinates all SNF admission and discharge procedures. Ensures compliance with federal, state and local regulations and established policies and procedures.

Ensures smooth transition of patients from other levels of care to Skilled Nursing Facility (SNF). Coordinates all SNF admission and discharge procedures. Ensures compliance with federal, state and local regulations and established policies and procedures.
Essential Responsibilities:
- Coordinates KPPACC patient admissions. Provides coordination of KPPACC patient admissions in compliance with federal, state and local regulations and established policies and procedures. Coordinates necessary supplies, equipment and materials to meet the needs of patients who are admitted to KPPACC.
- Obtain all authorizations/approvals as needed for patients/families. Acts as resource to patients, family, and agencies. Interfaces with other department heads in resolving member grievances regarding admissions. Reports findings to quality improvement committee, or other committees as indicated.
- Collects, coordinates, and analyzes data related to admissions. Participates in case review and continuum of care discussions for those patients identified as potential admissions to KPPACC.
- Assesses patients needs and the type of SNF care required. Interprets SNF policies, regulations and procedures applicable to patients, caregivers, physicians, health care providers and external agencies.
- Maintains an accurate facility census for all facility departments. Explains facility philosophy, policies and procedures to potential and new residents and residents representatives.
- Assists manager in developing and implementing SNF policies and procedures. Assists in updating and expanding Admissions protocols.
- Coordinates the referrals process, reviews transfer and admission paperwork. Coordinates effective communication, and plans proper utilization.
- Kaiser Permanente conducts compensation reviews of positions on a routine basis. At any time, Kaiser Permanente reserves the right to reevaluate and change job descriptions, or to change such positions from salaried to hourly pay status. Such changes are generally implemented only after notice is given to affected employees.
