Job Summary:
The LPN, practices within scope of practice perimeters as inferred by licensure, standard nursing practice, knowledge, skill level, and organizational policies. The LPN provides support for team and providers(s) during facility encounters. Performs follow-up care per provider instructions and in accordance with scope of practice. Assists with patient education and documents clinical information in the medical record. Assists in the planning and delivery of nursing services for patients, members and families to promote health and wellness. Provides direct nursing care, demonstrating clinical competency by performing various roles as needed, providing advanced levels of nursing care and proactively monitoring and delivering preventive/early intervention and/or education programs to meet patient needs. Collaborates with the Health Care Team to deliver high quality, safe care and service. Facilitates transitions and coordination of care across the continuum.

Essential Responsibilities:
- The LPN, is responsible for accurate completion of screenings as reflected in the workflow by addressing barriers with focus on care gaps and patient goals. Schedules appointments within the appropriate discipline based on proactive care needs.
- Assists with teaching/education of patient as assigned based on national guidelines specific to the screening test results. Teaches and promotes general preventive health measures and refers to appropriate resources.
- Reviews client consent and other forms for completeness and legibility before offering services.
- Accurately documents patient information, data, actions, interventions, and patient response in according to established guidelines and policies.
- Prepares room and equipment for procedures, assists provider as needed. Provides intervention ordered by practitioner (e.g. medication, dressings, specimen collections, irrigations, suture removals, and IVs) and documents patient response.
- Assist with event setting / closing; ensures all equipment and forms are stored and tracked appropriately. Ensures a clean and functional work environment.
- Reviews patient history, collecting information and appropriate vital signs, administers immunizations, obtains biometrics, etc. as needed.
- Participates in emergency care as necessary and as directed by physician and registered nurse.
- Responds effectively to member / patient concerns, serves as an advocate for the patient.
- Demonstrates customer service skills to include; diplomacy, patience, problem solving with a range of clientele.
- Adjusts to fluctuating work flows and changing assignments; is expected to demonstrate quality and effectiveness in work habits and clinical practice; and treats co-workers, patients, families, and all members of the health care team with dignity and respect.
- Able to provide clinical services in a variety of settings (i.e. clinic, employer group sites, mobile health vehicle, etc.).
- Participates in quality initiatives (HEDIS, CMS) and leads designated team projects or initiatives as requested.
- Assures team/departments compliance with regulatory agency requirements and federal, state, local laws and regulations.
- Conducts outgoing telephone calls to patients requiring preventative outreach.
- Completes other department-specific procedures and nursing care within scope of practice and skill.
- Assists with event logistics and coordination such as but not limited to inventory, supplies availability, event tracking and reporting.
- Participates in the development and maintenance of data collections and report requirements.
- Attends and participates in departmental meetings as required and/or requested.
- Completes required staff development and other trainings as requested.
- Participates in fostering a safe workplace: participates in employee job safety analysis to eliminate hazards and prevent accidents, cooperates to investigate employee and volunteer injuries, promotes and support safety awareness and implement safety operations, encourages staff to make suggestions on safety practices.
- Participates in education/orientation of other staff.
