Job Summary:
To provide cohesive, consistent, and comprehensive care to the Pediatric Diabetic population through the coordination and management of multidisciplinary teams at multiple clinical sites. To improve outcomes for the Pediatric Diabetic population by providing Case Management and diabetic educational services.
Essential Responsibilities:
To provide cohesive, consistent, and comprehensive care to the Pediatric Diabetic population through the coordination and management of multidisciplinary teams at multiple clinical sites. To improve outcomes for the Pediatric Diabetic population by providing Case Management and diabetic educational services.
Essential Responsibilities:
- Design, implement, and maintain the clinical and educational programs/activities for Peds diabetic patients in the NW region.
- Coordinate and manage the multidisciplinary diabetes team for consistent and comprehensive standards of care for Peds diabetic patients. Coordinates the actions of the multidisciplinary team with a diverse set of goals and clinical backgrounds.
- Develop, implement, monitor and analyze patient outcome and quality assurance measures to assure effectiveness of overall program. Assures that the program meets community and national standards (i.e., HEIDS guidelines).
- Provide direct care to members and families including Case Management of chronically ill patients with varying social, economic, emotional, developmental and medical issues which may affect compliance with the plan of care.
- Assess, plan, implement, and evaluate effectiveness of individual patient care plans based on the nursing process. Develop and apply nursing protocols as deemed appropriate by the diabetes team. Provides assessments, telephone advice, medication adjustments and patient instruction using protocols and knowledge unique to the CDE skill set (i.e., insulin teaching, insulin pump training appraised of clinical best practices based on research driven evidence.
- Coordination of the administrative functions associated with providing multidisciplinary services to patients at five (5) clinical sites. Maintaining effective communication linkage with five (5) clinical sites and team members.
