Job Summary:
Plans and supervises Long Term Care (LTC) staff and budget. Ensures cost effective operation with maximum efficiency while maintaining high level of quality and safe care and service throughout Region. Plans, develops, implements, supervises, and evaluates Skilled Nursing Facility (SNF)/LTC care coordination which may include DME, referrals and elder care to enhance admission, discharge/transfer to most timely, appropriate setting. Promotes continuity of care and work with other SNFs to ensure quality and safety of care, utilization and implementation of benefits. Develops, implements and maintains clinical practice standards and alternative care options to prevent admissions to acute hospital. Collaborates with appointed medical staff, to ensure UM Program meets regulatory requirements. Participates in UM strategic planning and Resource Management-UM Committee. Evaluates and effectively markets services. Assists with accreditation, certification and licensing preparedness.

Essential Responsibilities:
- Establishes, implements and maintains patient care and service standards to meet residents expectations.
- Designs, and evaluates processes, policies and procedures to improve systems and patient care outcomes/results across the continuum of care. Recommends changes and implements policies and procedures. Ensures compliance of same with regulations.
- Ensures compliance in meeting all Federal and State regulations and standards related to Long-Term Care/SNF, i.e. CMS
- Reviews educational needs of staff with peers/staff/educators. Facilitates ongoing staff development and ensures competent staff.
- Directs and coordinates team efforts to optimize skilled days, including effective admission determinations, discharge planning, utilization of services including ancillary services and timely placement.
- Participates in utilization and multidisciplinary rounds for timely, appropriate medical treatment. Evaluates plan of care. Communicates admission and discharges to administrative staff and others as appropriate.
- Assists in development of and monitors and controls payroll and non-payroll budget for LTC area to provide efficient, cost-effective patient services.
- Coordinates statistical data and presents reports.
- Supports Director of Nursing in accomplishment of strategic goals. Collaborates with Director of Nursing to develop SNF/LTC
- Utilization targets that can be converted into fiscal savings.
- Ensures ongoing effectiveness of quality management program for the department to include selection of key processes and indicators of quality, monitoring, and evaluating improvement.
- Establishes optimum working relationships with chiefs of service, physicians, Extended Care Coordinators. Case Managers and staff of other systems SNFs staff and management to effectuate cost containment objectives.
- Encourages positive working environment.
- Ensures appropriate utilization of resources and provision of quality care to residents.
- Coordinates extended care reviews, benefit eligibility and statistical data.
- Liaisons with external review organizations. Researches questionable requirements which affect procedures. Updates staff on current changes in requirements by intermediaries.
- May perform patient care to the extent necessary to maintain clinical expertise, competency, and licensing necessary to fulfill job responsibilities and to direct the provision of care on the unit.
- Provides direct patient care on an as needed basis. Provides services that are within scope of license and in compliance with all legal, regulatory, and policy requirements relevant to clinical role performed.
- Hires, trains, supervises, counsels, disciplines, and terminates assigned staff as appropriate.
- Communicates goals, objectives, accountabilities, priorities, and authority parameters to assigned staff.
